mm/hwpoison: add MODULE_DESCRIPTION()
Patch series "mm: add missing MODULE_DESCRIPTION() macros".
This fixes the instances of "WARNING: modpost: missing
MODULE_DESCRIPTION()" that I'm seeing in mm/.
