pcnet_cs: add cis of National Semicondoctor's multifunction pcmcia card
pcnet_cs,serial_cs:
add cis of National Semicondoctor's lan&modem mulitifunction pcmcia card,
NE2K, tamarack ethernet card,
and some serial card(COMpad2, COMpad4).
