The selection and scroll bar are really needed only when the user starts
navigating, before that it just provide distractions.
This also brings the initial screen to look more like the stdio UI,
which more people are used to.
The new code is flexible enough that menu like browsers can opt out and
start with those UI elements.

Its becoming common to allow the user to filter out parts of the data
structure being browsed, like already done in the hists browser and in
the annotate browser in the next commit, so provide it directly in the
ui_browser class list_head helpers.
More work required to move the equivalent routines found now in the
hists browser to the rb_tree helpers.

With underlying dynamic data structures we need to invalidate pointers
to them after a timer, as that entry may have vanished (decayed in top,
for instance).
I forgot about browser_ui->top. Fix it by resetting it to null after a
timer. The seek operation from SEEK_SET will then set it to a valid
entry because it starts from rb_first(&hists->entries).

When using multiple events the 'top' and 'report' tools will first
present the user with a menu to choose the event to browse.
After that the user can either press <- to go back to the menu and
choose another event or instead press TAB to go the next event without
having to go back to the menu or shift-TAB (UNTAB) to go the previous
event, useful to quickly visually see if multiple events are correlated.
The handling of each hists browser return was broken by the ed7e566,
that combined both switches, the first that was for choosing the event
and the second that was for checking if switching to the next event
without passing thru the events menu.
Repeat with me: Don't be clever like that.
Fix it by moving the switch to right after the call to the hists
browser, making abundantly clear that the two switches are unrelated.
This also fixes a compiler warning about the 'pos' variable being
possibly used unitialized.

To do that we needed to stop using newtForm, as we don't want libnewt to
catch the xterm resize signal.
Remove some more newt calls and instead use the underlying libslang
directly. In time tools/perf will use just libslang.

In browsers that access dynamic underlying data structures, like in the
hists browser and its hist_entry rb_tree, we need to revalidate any
reference to the underlying data structure, because they can have gone
away, decayed.
This fixes a problem where after a while the top entries get behind the
top of the screen, i.e. the top_idx stays at 0, which means it is at the
first entry in the rb_tree when in fact it wasn't because the
browser->top didn't got revalidated after the timer ran and the
underlying data structure got updated.

Previously the hist_browser dealt with a static tree of entries, now it
needs to update the nr_entries in the browser after the timer runs.
A better solution will come when moving using another thread for the
collapse_resort, etc, but for now this is ok.

When requesting multiple events, say:
# perf top -e instructions -e cycles -e cache-misses
The first screen lets the user chose what to see first, then to switch
one can either use the left key to get back to the event menu or simply
use TAB to go the next and shift+TAB to go the prev.
When using TAB/UNTAB the call to perf_evlist__set_selected(event) was
missing, fix it.

I.e. when in the annotate TUI window, if Enter is pressed over an
assembly line with a 'callq' it will try to open another TUI window with
that symbol.
This is just a proof of concept and works only on x86_64, more work is
needed to support kernel modules, userland, other arches, etc, but
should already be useful as-is.

This actually fixes several problems we had in the old 'perf top':
1. Unresolved symbols not show, limitation that came from the old
"KernelTop" codebase, to solve it we would need to do changes
that would make sym_entry have most of the hist_entry fields.
2. It was using the number of samples, not the sum of sample->period.
And brings the --sort code that allows us to have all the views in
'perf report', for instance:

This allows passing a timer to be run periodically, which will update
the hists tree that then gers refreshed on the screen, just like the
Live mode (symbol entries, annotation) we already have in 'perf top
--tui'.

In hists browser, press hotkey 'a' to annotate current symbol.
Now it causes segment fault if 'a' is pressed on a null symbol.
Here are 2 small bugs:
- In perf_evsel__hists_browse, the condition check after 'a' is pressed
is not correct, we should check ->sym instead of ->map.
- In symbol__tui_annotate we must check whether sym is NULL or not
before getting annotation structure.
This patch fixes above 2 small bugs.

When multiple events were used in 'perf record', allow the user to
choose which one is wanted before showing the per event histograms.
Annotations will be performed on the chosen event.
Allow going back and forth from event to event quickly using just the
arrow keys and enter.

By creating an perf_evlist out of the attributes in the perf.data file
header, so that we can use evlists and evsels when reading recorded
sessions in addition to when we record sessions.
More work is needed to allow tools to allow the user to select which
events are wanted when browsing sessions, be it just one or a subset of
them, aggregated or showed at the same time but with different
indications on the UI to allow seeing workloads thru different views at
the same time.
But the overall goal/trend is to more uniformly use evsels and evlists.

By taking the ui__lock so that no other screen updates take place while
waiting for the user.
That was happening when handling an invalid --vmlinux parameter in 'perf
top --tui', with the screen refresh routine repainting the screen and
removing the warning window.

Needed because we were only showing the title in ui_browser__show,
not in ui_browser__run, and in the run loop we may be calling other
browsers that would then change the title, when we go back to the
previous browser, we need to redraw the title.
We could have done this as the Newt help line, with pop, etc, but I
don't think its worth, doing it explicitely, when needed (some browsers
may not use the title area at all) seems enough/more flexible.

The left key was exiting 'perf top --tui' when it really shouldn't, it
was too easy to leave the live annotation window and then press one too
many <- and get out of the tool altogether.
Do just like the report TUI does, ignore the left key for exit and also
ask the user when pressing ESC if that is really what is wanted.

This is not a problem when we're not at the bottom of the active symbols
list, so was not noticed, but at the end of the screen it falls apart.
Fix it by adjusting the ui_browser indexes according to the new number
of entries in the rb_tree and by seeking from the start of the rb_tree
to find the new symbol at the top of the screen.

Now one has just to press the right key, 'a' or Enter on the main 'perf
top --tui' screen to live annotate the symbol under the cursor.
The annotate window starts centered on the hottest line (the one with
most samples so far) then TAB and shift+TAB can be used to go to the
prev/next hot line.
Pressing 'H' at any point will center again the screen on the hottest
line.

The ui operations so far were used by just one thread, but 'perf top
--tui' now has two threads updating the screen, so we need to use a
mutex to avoid garbling the screen.

Since we'll need it when implementing the live annotate TUI browser.
This also simplifies things a bit by having the list head for the source
code to be in the dynamicly allocated part of struct annotation, that
way we don't have to pass it around, it can be found from the struct
symbol that is passed everywhere.

GCC 4.6.0 in Fedora rawhide turned up some compile errors in tools/perf
due to the -Werror=unused-but-set-variable flag.
I've gone through and annotated some of the assignments that had side
effects (ie: return value from a function) with the __used annotation,
and in some cases, just removed unused code.
In a few cases, we were assigning something useful, but not using it in
later parts of the function.

histograms, but to support the top model add support for one histogram
perf evsel in the evlist.

They will be used by perf top, so that we have just one set of routines
to do annotation.
Rename "struct sym_priv" to "struct annotation", etc, to clarify this
code a bit.
Rename "struct sym_ext" to "struct source_line", to give it a meaningful
name, that clarifies that it is a the result of an addr2line call, that
is sorted by percentage one particular source code line appeared in the
annotation.
And since we're moving things around also rename 'sym_hist->ip' to
'sym_hist->addr' as we want to do data structure annotation at some
point.

entry.

Using %L[uxd] has issues in some architectures, like on ppc64. Fix it
by making our 64 bit integers typedefs of stdint.h types and using
PRI[ux]64 like, for instance, git does.

Fix it by explaining what can be happening and giving the number of processed
and lost events.
Also holler if unknown events were found, that can be due to processing a
perf.data file collected using a newer tool where newer events got added on
reporting using an older perf tool, that or a bug, so ask for a report to be
made.
Works on both --tui and --stdio.

The gcc complains about small auto-var strings being allocated from stack space.
Make them const to avoid this:
