On tui annotation, the title was set to name of the target symbol if
user selects the target. However it remained after returning to original
symbol from the target. Fix it.

Accepting upper case character only is unconvenient since it requires
SHIFT key too. Why not change to it accept a simple key stroke?

When the user navigates to another annotation browser pressing -> on a
'callq' line, on exit (<-) return to the originating 'callq' line.

The selection and scroll bar are really needed only when the user starts
navigating, before that it just provide distractions.
This also brings the initial screen to look more like the stdio UI,
which more people are used to.
The new code is flexible enough that menu like browsers can opt out and
start with those UI elements.

while the current line remains selected. Press 'S' again to show them

To do that we needed to stop using newtForm, as we don't want libnewt to
catch the xterm resize signal.
Remove some more newt calls and instead use the underlying libslang
directly. In time tools/perf will use just libslang.

I.e. when in the annotate TUI window, if Enter is pressed over an
assembly line with a 'callq' it will try to open another TUI window with
that symbol.
This is just a proof of concept and works only on x86_64, more work is
needed to support kernel modules, userland, other arches, etc, but
should already be useful as-is.

This allows passing a timer to be run periodically, which will update
the hists tree that then gers refreshed on the screen, just like the
Live mode (symbol entries, annotation) we already have in 'perf top
--tui'.
Will be used by the new hist_entry/hists based 'top' tool.

In hists browser, press hotkey 'a' to annotate current symbol.
Now it causes segment fault if 'a' is pressed on a null symbol.
Here are 2 small bugs:
- In perf_evsel__hists_browse, the condition check after 'a' is pressed
is not correct, we should check ->sym instead of ->map.
- In symbol__tui_annotate we must check whether sym is NULL or not
before getting annotation structure.
This patch fixes above 2 small bugs.

Now one has just to press the right key, 'a' or Enter on the main 'perf
top --tui' screen to live annotate the symbol under the cursor.
The annotate window starts centered on the hottest line (the one with
most samples so far) then TAB and shift+TAB can be used to go to the
prev/next hot line.
Pressing 'H' at any point will center again the screen on the hottest
line.

Since we'll need it when implementing the live annotate TUI browser.
This also simplifies things a bit by having the list head for the source
code to be in the dynamicly allocated part of struct annotation, that
way we don't have to pass it around, it can be found from the struct
symbol that is passed everywhere.

They will be used by perf top, so that we have just one set of routines
to do annotation.
Rename "struct sym_priv" to "struct annotation", etc, to clarify this
code a bit.
Rename "struct sym_ext" to "struct source_line", to give it a meaningful
name, that clarifies that it is a the result of an addr2line call, that
is sorted by percentage one particular source code line appeared in the
annotation.
And since we're moving things around also rename 'sym_hist->ip' to
'sym_hist->addr' as we want to do data structure annotation at some
point.

This makes the usual idiom for specifying a series of key codes to exit
ui_browser__run() for specialized processing (search, annotate, etc) or
plain exiting the browser more compact.
It also abstracts away some more libnewt operations. At some point we'll
also replace NEWT_KEY_foo with something that can be mapped to NEWT or,
say, gtk.

Browsers don't have to deal with absolute coordinates, just using (row,
column) and leaving the rest to ui_browser is better and removes one
more UI backend detail from the browsers.
Also shorten the percent_color setting idiom, removing some more direct
libslang calls.

As part of ongoing effort to reduce the coupling with libnewt, browsers
are being changed to return the exit key.
The annotate browser is not returning it as expected by builtin-annotate
when annotating multiple symbols (when 'perf annotate' is called without
specifying a symbol name).
Fix it by returning the exit key and also adding the RIGHT key as a exit
key so that going to the next symbol in the TUI can work again.

So that the common tasks of providing a helpline at __run entry and
destroying the window and releasing resourses at exit can be abstracted

The annotate TUI now starts centered on the line with most samples, i.e.
the hottest line in the annotated function. Pressing TAB will center on
the second hottest function and so on. Shift+TAB goes in the other
direction.
This way one can more easily sift thru the function hotspots.
