Remove the "struct mutex lock" variable from annotation that is
allocated per symbol. This removes in the region of 40 bytes per
symbol allocation. Use a sharded mutex where the number of shards is
set to the number of CPUs. Assuming good hashing of the annotation
(done based on the pointer), this means in order to contend there
needs to be more threads than CPUs, which is not currently true in any
perf command. Were contention an issue it is straightforward to
increase the number of shards in the mutex.
On my Debian/glibc based machine, this reduces the size of struct
annotation from 136 bytes to 96 bytes, or nearly 30%.

hists__find_annotations() allows to move to next or previous symbols for
annotation using the arrow keys. But TUI annotate_browser__run() uses
the RIGHT key as ENTER to handle jump/call instructions. That makes the
navigation to the next function impossible.
I'd like to change it back to move the next symbol but I'm afraid if
some users get confused. So I added a new pair of keys to handle that.

Later changes will add reference count checking for struct map, with
dso being the most frequently accessed variable. Add an accessor so
that the reference count check is only necessary in one place.
Additional changes:
- add a dso variable to avoid repeated map__dso calls.
- in builtin-mem.c dump_raw_samples, code only partially tested for
dso == NULL. Make the possibility of NULL consistent.
- in thread.c thread__memcpy fix use of spaces and use tabs.

Handle 'f' key to toggle the display offset and full address. Obviously
it only works when users set to see disassembler output ('o' key). It'd
be useful when users want to see the full virtual address in the TUI
annotate browser.

recursive functions
In 'perf report', entering a recursive function from inside of itself
(either directly of indirectly through some other function) results in
calling symbol__annotate2 multiple() times, and freeing the whole
disassembly when exiting from the innermost instance.
The first issue causes the function's disassembly to be duplicated, and
the latter a heap use-after-free (and crash) when trying to access the
disassembly again.
I reproduced the bug on perf 5.11.22 (Ubuntu 20.04.3 LTS) and 5.16.rc8
with the following testcase (compile with gcc recursive.c -o recursive).
To reproduce:
- perf record ./recursive
- perf report
- enter fibonacci and annotate it
- move the cursor on one of the "callq fibonacci" instructions and press enter
- at this point there will be two copies of the function in the disassembly
- go back by pressing q, and perf will crash

This patch addresses the issue by annotating a function and freeing the
associated memory on exit only if no annotation is already present, so
that a recursive function is only annotated on entry.

Some x86 microarchitectures fuse a subset of cmp/test/ALU instructions
with branch instructions, and thus perf annotate highlight such valid
pairs as fused.
When annotated with source, perf uses struct disasm_line to contain
either source or instruction line from objdump output. Usually, a C
statement generates multiple instructions which include such
cmp/test/ALU + branch instruction pairs. But in case of assembly
function, each individual assembly source line generate one
instruction.
The 'perf annotate' instruction fusion logic assumes the previous
disasm_line as the previous instruction line, which is wrong because,
for assembly function, previous disasm_line contains source line. And
thus perf fails to highlight valid fused instruction pairs for assembly
functions.
Fix it by searching backward until we find an instruction line and
consider that disasm_line as fused with current branch instruction.

Setting annotate_warned to true on errors was removed in
commit ee51d851392e ("perf annotate: Introduce strerror for handling
symbol__disassemble() errors") which means when 'perf annotate
--skip-missing' is used warnings are shown multiple times for the same
DSO.
Setting this again restores the original behavior of only one warning
each.

Move evsel::idx to perf_evsel::idx, so we can move the group interface
to libperf.
Committer notes:

If the disasm is empty, 's' should fail. Instead it seemingly works,
hiding the empty lines and causing an assertion error on the next time
annotate is called (from within perf report).
The problem is caused by a buffer overflow, caused by a wrong exit
condition in annotate_browser__find_next_asm_line, which checks
browser->b.top instead of browser->b.entries.
This patch fixes the issue, making annotate_browser__toggle_source
fail if the disasm is empty (nothing happens to the user).

In perf annotate, when 's' is pressed on a line containing source code,
it shows the message "Only available for assembly lines".
This patch gets rid of the error, moving the cursr to the next available
asm line (or the closest previous one if no asm line is found moving
forwards), before hiding source code lines.
Changes in v2:
- handle case of no asm line found in
annotate_browser__find_next_asm_line by returning NULL and
handling error in caller.

Right now, when Line numbers are displayed, one can't easily find a
source file that the line corresponds to.
When a source line is selected and 'l' is pressed, full source file
location is displayed in perf UI footer line. The hotkey works only for
source code lines.

perf annotate --show-nr-samples does not really show number of samples.
The reason is we have two separate variables for the same purpose.
One is in symbol_conf.show_nr_samples and another is
annotation_options.show_nr_samples.
We save command line option in symbol_conf.show_nr_samples but uses
annotation_option.show_nr_samples while rendering tui/stdio2 browser.
Though, we copy symbol_conf.show_nr_samples to
annotation__default_options.show_nr_samples but that is not really
effective as we don't use annotation__default_options once we copy
default options to dynamic variable annotate.opts in cmd_annotate().
Instead of all these complication, keep only one variable and use it all
over. symbol_conf.show_nr_samples is used by perf report/top as well. So
let's kill annotation_options.show_nr_samples.
On a side note, I've kept annotation_options.show_nr_samples definition
because it's still used by perf-config code. Follow up patch to fix
perf-config for annotate will remove annotation_options.show_nr_samples.

The 'perf annotate' TUI browser provides a 'r' hot key to switch to a

'symbol' pointers
We are already passing things like:
symbol__annotate(ms->sym, ms->map, ...)
So shorten the signature of such functions to receive the 'map_symbol'
pointer.
This also paves the way to having the 'struct map_groups' pointer in the
'struct map_symbol' so that we can get rid of 'struct map'->groups.

Some compilers will complain when using a member of a struct to
initialize another member, in the same struct initialization.
For instance:

The scripts menu traditionally only showed custom perf scripts.
Allow to run standard perf script with useful default options too.
- Normal perf script
- perf script with assembler (needs xed installed)
- perf script with source code output (needs debuginfo)
- perf script with custom arguments
Then we automatically select the right options to display the
information in the perf.data file.
For example with -b display branch contexts.
It's not easily possible to check for xed's existence in advance. perf
script usually gives sensible error messages when it's not available.

The annotation line percentage is compared and inserted into the rbtree,
but the percent field of 'struct annotation_data' is an array, the
comparison result between them is the address difference.
This patch compares the right slot of percent array according to
opts->percent_type and makes things right.
The problem can be reproduced by pressing 'H' in perf top annotation view.
It should highlight the instruction line which has the highest sampling
percentage.

So we can hold multiple percent values for annotation line.
The first member of this array is current local hits percent value
(PERCENT_HITS_LOCAL index), so no functional change is expected.
Adding annotation_data__percent function to return requested percent
value from struct annotation_data.

The name 'samples*' is little confusing because we have nested 'struct
sym_hist_entry' under annotation_line struct, which holds 'nr_samples'
as well.
Also the holding struct name is 'annotation_data' so the 'data' name
fits better.

We have more current function tto get the title for annotation,
which is hists__scnprintf_title. They both have same output as
far as the annotation's header line goes.
They differ in counting of the nr_samples, hists__scnprintf_title
provides more accurate number based on the setup of the
symbol_conf.filter_relative variable.
Plus it also displays any uid/thread/dso/socket filters/zooms
if there are set any, which annotation__scnprintf_samples_period
does not.

Now that we can have extra title lines we should use ui_browser->rows
and not ->height when drawing lines, as it will use ui_browser__gotorc()
and that will take the extra title lines into account, which was causing
an off by one at the end of the vertical line drawn by
__ui_browser__vline(), fix it.
The visual effect was that the last line, with status messages, was
being overwritten by the vertical line, looking like:

Because they all really check if we can access data structures/visual
constructs where a "jump" instruction targets code in the same function,
i.e. things like:

Just like we have in the histograms browser used as the main screen for
'perf top --tui' and 'perf report --tui', to print the current
annotation to a file with a named composed by the symbol name and the
".annotation" suffix.
Here is one example of pressing 'A' on 'perf top' to live annotate a
kernel function and then press 'P' to dump that annotation, the
resulting file:
