Pull MM updates from Andrew Morton:
- Yu Zhao's Multi-Gen LRU patches are here. They've been under test in
linux-next for a couple of months without, to my knowledge, any
negative reports (or any positive ones, come to that).
- Also the Maple Tree from Liam Howlett. An overlapping range-based
tree for vmas. It it apparently slightly more efficient in its own
right, but is mainly targeted at enabling work to reduce mmap_lock
contention.
Liam has identified a number of other tree users in the kernel which
could be beneficially onverted to mapletrees.
Yu Zhao has identified a hard-to-hit but "easy to fix" lockdep splat
at [1]. This has yet to be addressed due to Liam's unfortunately
timed vacation. He is now back and we'll get this fixed up.
- Dmitry Vyukov introduces KMSAN: the Kernel Memory Sanitizer. It uses
clang-generated instrumentation to detect used-unintialized bugs down
to the single bit level.
KMSAN keeps finding bugs. New ones, as well as the legacy ones.
- Yang Shi adds a userspace mechanism (madvise) to induce a collapse of
memory into THPs.
- Zach O'Keefe has expanded Yang Shi's madvise(MADV_COLLAPSE) to
support file/shmem-backed pages.

After the ucount rlimit code was merged a bunch of small but
siginificant bugs were found and fixed. At the time it was realized
that part of the problem was that while the ucount rlimits were very
similar to the oridinary ucounts (in being nested counts with limits)
the semantics were slightly different and the code would be less error
prone if there was less sharing.
This is the long awaited cleanup that should hopefully keep things
more comprehensible and less error prone for whoever needs to touch
that code next"

Commit 2e13ba54a268 ("fs, proc: introduce CONFIG_PROC_CHILDREN")
introduces the config PROC_CHILDREN to configure kernels to provide the
/proc/<pid>/task/<tid>/children file.
When one deselects PROC_FS for kernel builds without /proc/, the config
PROC_CHILDREN has no effect anymore, but is still visible in menuconfig.
Add the dependency on PROC_FS to make the PROC_CHILDREN option disappear
for kernel builds without /proc/.

Patch series "ksm: count allocated rmap_items and update documentation",
v5.
KSM can save memory by merging identical pages, but also can consume
additional memory, because it needs to generate rmap_items to save each
scanned page's brief rmap information.
To determine how beneficial the ksm-policy (like madvise), they are using
brings, so we add a new interface /proc/<pid>/ksm_stat for each process
The value "ksm_rmap_items" in it indicates the total allocated ksm
rmap_items of this process.
The detailed description can be seen in the following patches' commit
message.
This patch (of 2):
KSM can save memory by merging identical pages, but also can consume
additional memory, because it needs to generate rmap_items to save each
scanned page's brief rmap information. Some of these pages may be merged,
but some may not be abled to be merged after being checked several times,
which are unprofitable memory consumed.
The information about whether KSM save memory or consume memory in
system-wide range can be determined by the comprehensive calculation of
pages_sharing, pages_shared, pages_unshared and pages_volatile. A simple
approximate calculation:
profit =~ pages_sharing * sizeof(page) - (all_rmap_items) *
sizeof(rmap_item);
where all_rmap_items equals to the sum of pages_sharing, pages_shared,
pages_unshared and pages_volatile.
But we cannot calculate this kind of ksm profit inner single-process wide
because the information of ksm rmap_item's number of a process is lacked.
For user applications, if this kind of information could be obtained, it
helps upper users know how beneficial the ksm-policy (like madvise) they
are using brings, and then optimize their app code. For example, one
application madvise 1000 pages as MERGEABLE, while only a few pages are
really merged, then it's not cost-efficient.
So we add a new interface /proc/<pid>/ksm_stat for each process in which
the value of ksm_rmap_itmes is only shown now and so more values can be
added in future.
So similarly, we can calculate the ksm profit approximately for a single
process by:
profit =~ ksm_merging_pages * sizeof(page) - ksm_rmap_items *
sizeof(rmap_item);
where ksm_merging_pages is shown at /proc/<pid>/ksm_merging_pages, and
ksm_rmap_items is shown in /proc/<pid>/ksm_stat.

By using the maple tree and the maple tree state, the vmacache is no
longer beneficial and is complicating the VMA code. Remove the vmacache
to reduce the work in keeping it up to date and code complexity.

We've got a bunch of special swap entries that stores PFN inside the swap
offset fields. To fetch the PFN, normally the user just calls
swp_offset() assuming that'll be the PFN.
Add a helper swp_offset_pfn() to fetch the PFN instead, fetching only the
max possible length of a PFN on the host, meanwhile doing proper check
with MAX_PHYSMEM_BITS to make sure the swap offsets can actually store the
PFNs properly always using the BUILD_BUG_ON() in is_pfn_swap_entry().
One reason to do so is we never tried to sanitize whether swap offset can
really fit for storing PFN. At the meantime, this patch also prepares us
with the future possibility to store more information inside the swp
offset field, so assuming "swp_offset(entry)" to be the PFN will not stand
any more very soon.
Replace many of the swp_offset() callers to use swp_offset_pfn() where
proper. Note that many of the existing users are not candidates for the
replacement, e.g.:
(1) When the swap entry is not a pfn swap entry at all, or,
(2) when we wanna keep the whole swp_offset but only change the swp type.
For the latter, it can happen when fork() triggered on a write-migration
swap entry pte, we may want to only change the migration type from
write->read but keep the rest, so it's not "fetching PFN" but "changing
swap type only". They're left aside so that when there're more
information within the swp offset they'll be carried over naturally in
those cases.
Since at it, dropping hwpoison_entry_to_pfn() because that's exactly what
the new swp_offset_pfn() is about.

MADV_COLLAPSE is not coupled to the kernel-oriented sysfs THP settings[1].
hugepage_vma_check() is the authority on determining if a VMA is eligible
for THP allocation/collapse, and currently enforces the sysfs THP
settings. Add a flag to disable these checks. For now, only apply this
arg to anon and file, which use /sys/kernel/transparent_hugepage/enabled.
We can expand this to shmem, which uses
/sys/kernel/transparent_hugepage/shmem_enabled, later.
Use this flag in collapse_pte_mapped_thp() where previously the VMA flags
passed to hugepage_vma_check() were OR'd with VM_HUGEPAGE to elide the
VM_HUGEPAGE check in "madvise" THP mode. Prior to "mm: khugepaged: check
THP flag in hugepage_vma_check()", this check also didn't check "never"
THP mode. As such, this restores the previous behavior of
collapse_pte_mapped_thp() where sysfs THP settings are ignored. See
comment in code for justification why this is OK.

We keep track of several kernel memory stats (total kernel memory, page
tables, stack, vmalloc, etc) on multiple levels (global, per-node,
per-memcg, etc). These stats give insights to users to how much memory
is used by the kernel and for what purposes.
Currently, memory used by KVM mmu is not accounted in any of those
kernel memory stats. This patch series accounts the memory pages
used by KVM for page tables in those stats in a new
NR_SECONDARY_PAGETABLE stat. This stat can be later extended to account
for other types of secondary pages tables (e.g. iommu page tables).
KVM has a decent number of large allocations that aren't for page
tables, but for most of them, the number/size of those allocations
scales linearly with either the number of vCPUs or the amount of memory
assigned to the VM. KVM's secondary page table allocations do not scale
linearly, especially when nested virtualization is in use.
From a KVM perspective, NR_SECONDARY_PAGETABLE will scale with KVM's
per-VM pages_{4k,2m,1g} stats unless the guest is doing something
bizarre (e.g. accessing only 4kb chunks of 2mb pages so that KVM is
forced to allocate a large number of page tables even though the guest
isn't accessing that much memory). However, someone would need to either
understand how KVM works to make that connection, or know (or be told) to
go look at KVM's stats if they're running VMs to better decipher the stats.
Furthermore, having NR_PAGETABLE side-by-side with NR_SECONDARY_PAGETABLE
is informative. For example, when backing a VM with THP vs. HugeTLB,
NR_SECONDARY_PAGETABLE is roughly the same, but NR_PAGETABLE is an order
of magnitude higher with THP. So having this stat will at the very least
prove to be useful for understanding tradeoffs between VM backing types,
and likely even steer folks towards potential optimizations.

These bits should only be valid when the ptes are present. Introducing
two booleans for it and set it to false when !pte_present() for both pte
and pmd accountings.
The bug is found during code reading and no real world issue reported, but
logically such an error can cause incorrect readings for either smaps or
smaps_rollup output on quite a few fields.
For example, it could cause over-estimate on values like Shared_Dirty,
Private_Dirty, Referenced. Or it could also cause under-estimate on
values like LazyFree, Shared_Clean, Private_Clean.

Function d_invalidate() will remove dentry from hash firstly, but why does
proc_flush_pid() process dentry '/proc/tgid/fd' before dentry
'/proc/tgid'? That's because proc_pid_make_inode() adds proc inode in
reverse order by invoking hlist_add_head_rcu(). But proc should not add
any inodes under '/proc/tgid' except '/proc/tgid/task/pid', fix it by
adding inode into 'pid->inodes' only if the inode is /proc/tgid or
/proc/tgid/task/pid.
Performance regression:
Create 200 tasks, each task open one file for 50,000 times. Kill all
tasks when opened files exceed 10,000,000 (cat /proc/sys/fs/file-nr).
Before fix:
$ time killall -wq aa
real 4m40.946s # During this period, we can see 'ps' and 'systemd'
taking high cpu usage.
After fix:
$ time killall -wq aa
real 1m20.732s # During this period, we can see 'systemd' taking
high cpu usage.

The page fault path checks THP eligibility with __transhuge_page_enabled()
which does the similar thing as hugepage_vma_check(), so use
hugepage_vma_check() instead.
However page fault allows DAX and !anon_vma cases, so added a new flag,
in_pf, to hugepage_vma_check() to make page fault work correctly.
The in_pf flag is also used to skip shmem and file THP for page fault
since shmem handles THP in its own shmem_fault() and file THP allocation
on fault is not supported yet.
Also remove hugepage_vma_enabled() since hugepage_vma_check() is the only
caller now, it is not necessary to have a helper function.

The transparent_hugepage_active() was introduced to show THP eligibility
bit in smaps in proc, smaps is the only user. But it actually does the
similar check as hugepage_vma_check() which is used by khugepaged. We
definitely don't have to maintain two similar checks, so kill
transparent_hugepage_active().
This patch also fixed the wrong behavior for VM_NO_KHUGEPAGED vmas.
Also move hugepage_vma_check() to huge_memory.c and huge_mm.h since it
is not only for khugepaged anymore.

With DEVICE_COHERENT, we'll soon have vm_normal_pages() return
device-managed anonymous pages that are not LRU pages. Although they
behave like normal pages for purposes of mapping in CPU page, and for COW.
They do not support LRU lists, NUMA migration or THP.
Callers to follow_page() currently don't expect ZONE_DEVICE pages,
however, with DEVICE_COHERENT we might now return ZONE_DEVICE. Check for
ZONE_DEVICE pages in applicable users of follow_page() as well.

Pss is the sum of the sizes of clean and dirty private pages, and the
proportional sizes of clean and dirty shared pages:
Private = Private_Dirty + Private_Clean
Shared_Proportional = Shared_Dirty_Proportional + Shared_Clean_Proportional
Pss = Private + Shared_Proportional
The Shared*Proportional fields are not present in smaps, so it is not
always possible to determine how much of the Pss is from dirty pages and
how much is from clean pages. This information can be useful for
measuring memory usage for the purpose of optimisation, since clean pages
can usually be discarded by the kernel immediately while dirty pages
cannot.
The smaps routines in the kernel already have access to this data, so add
a Pss_Dirty to show it to userspace. Pss_Clean is not added since it can
be calculated from Pss and Pss_Dirty.

Currently it requires poking at debugfs to figure out the size and
population of the zswap cache on a host. There are no counters for reads
and writes against the cache. As a result, it's difficult to understand
zswap behavior on production systems.
Print zswap memory consumption and how many pages are zswapped out in
/proc/meminfo. Count zswapouts and zswapins in /proc/vmstat.

Since the semantics of maximum rlimit values are different, it would be
better not to mix ucount and rlimit values. This will prevent the error
of using inc_count/dec_ucount for rlimit parameters.
This patch also renames the functions to emphasize the lack of
connection between rlimit and ucount.

This requires the pagemap code to be able to recognize the newly
introduced swap special pte for uffd-wp, meanwhile the general case for
hugetlb that we recently start to support. It should make pagemap uffd-wp
support complete.

When a process exits, /proc/${pid}, and /proc/${pid}/net dentries are
flushed. However some leaf dentries like /proc/${pid}/net/arp_cache
aren't. That's because respective PDEs have proc_misc_d_revalidate() hook
which returns 1 and leaves dentries/inodes in the LRU.
Force revalidation/lookup on everything under /proc/${pid}/net by
inheriting proc_net_dentry_ops.

The file permissions on the fdinfo dir from were changed from
S_IRUSR|S_IXUSR to S_IRUGO|S_IXUGO, and a PTRACE_MODE_READ check was added
for opening the fdinfo files [1]. However, the ptrace permission check
was not added to the directory, allowing anyone to get the open FD numbers
by reading the fdinfo directory.
Add the missing ptrace permission check for opening the fdinfo directory.

Creating a new netdevice allocates at least ~50Kb of memory for various
kernel objects, but only ~5Kb of them are accounted to memcg. As a result,
creating an unlimited number of netdevice inside a memcg-limited container
does not fall within memcg restrictions, consumes a significant part
of the host's memory, can cause global OOM and lead to random kills of
host processes.
The main consumers of non-accounted memory are:
~10Kb 80+ kernfs nodes
~6Kb ipv6_add_dev() allocations
6Kb __register_sysctl_table() allocations
4Kb neigh_sysctl_register() allocations
4Kb __devinet_sysctl_register() allocations
4Kb __addrconf_sysctl_register() allocations
Accounting of these objects allows to increase the share of memcg-related
memory up to 60-70% (~38Kb accounted vs ~54Kb total for dummy netdevice
on typical VM with default Fedora 35 kernel) and this should be enough
to somehow protect the host from misuse inside container.
Other related objects are quite small and may not be taken into account
to minimize the expected performance degradation.
It should be separately mentonied ~300 bytes of percpu allocation
of struct ipstats_mib in snmp6_alloc_dev(), on huge multi-cpu nodes
it can become the main consumer of memory.
This patch does not enables kernfs accounting as it affects
other parts of the kernel and should be discussed separately.
However, even without kernfs, this patch significantly improves the
current situation and allows to take into account more than half
of all netdevice allocations.

Patch series "Convert vmcore to use an iov_iter", v5.
For some reason several people have been sending bad patches to fix
compiler warnings in vmcore recently. Here's how it should be done.
Compile-tested only on x86. As noted in the first patch, s390 should take
this conversion a bit further, but I'm not inclined to do that work
myself.
This patch (of 3):
Instead of passing in a 'buf' and 'userbuf' argument, pass in an iov_iter.
s390 needs more work to pass the iov_iter down further, or refactor, but
I'd be more comfortable if someone who can test on s390 did that work.
It's more convenient to convert the whole of read_from_oldmem() to take an
iov_iter at the same time, so rename it to read_from_oldmem_iter() and add
a temporary read_from_oldmem() wrapper that creates an iov_iter.
