In the commit 1c96dcceaeb3
("KVM: x86: fix apic_accept_events vs check_nested_events"),
we accidently started latching SIPIs that are received while the cpu is not
waiting for them.
This causes vCPUs to never enter a halted state.

Centralize handling of interrupts from the userspace APIC
in kvm_cpu_has_extint and kvm_cpu_get_extint, since
userspace APIC interrupts are handled more or less the
same as ExtINTs are with split irqchip. This removes
duplicated code from kvm_cpu_has_injectable_intr and
kvm_cpu_has_interrupt, and makes the code more similar
between kvm_cpu_has_{extint,interrupt} on one side
and kvm_cpu_get_{extint,interrupt} on the other.

vmx_apic_init_signal_blocked is buggy in that it returns true
even in VMX non-root mode. In non-root mode, however, INITs
are not latched, they just cause a vmexit. Previously,
KVM was waiting for them to be processed when kvm_apic_accept_events
and in the meanwhile it ate the SIPIs that the processor received.
However, in order to implement the wait-for-SIPI activity state,
KVM will have to process KVM_APIC_SIPI in vmx_check_nested_events,
and it will not be possible anymore to disregard SIPIs in non-root
mode as the code is currently doing.
By calling kvm_x86_ops.nested_ops->check_events, we can force a vmexit
(with the side-effect of latching INITs) before incorrectly injecting
an INIT or SIPI in a guest, and therefore vmx_apic_init_signal_blocked
can do the right thing.

On successful nested VM-Enter, check for pending interrupts and convert
the highest priority interrupt to a pending posted interrupt if it
matches L2's notification vector. If the vCPU receives a notification
interrupt before nested VM-Enter (assuming L1 disables IRQs before doing
VM-Enter), the pending interrupt (for L1) should be recognized and
processed as a posted interrupt when interrupts become unblocked after
VM-Enter to L2.
This fixes a bug where L1/L2 will get stuck in an infinite loop if L1 is
trying to inject an interrupt into L2 by setting the appropriate bit in
L2's PIR and sending a self-IPI prior to VM-Enter (as opposed to KVM's
method of manually moving the vector from PIR->vIRR/RVI). KVM will
observe the IPI while the vCPU is in L1 context and so won't immediately
morph it to a posted interrupt for L2. The pending interrupt will be
seen by vmx_check_nested_events(), cause KVM to force an immediate exit
after nested VM-Enter, and eventually be reflected to L1 as a VM-Exit.
After handling the VM-Exit, L1 will see that L2 has a pending interrupt
in PIR, send another IPI, and repeat until L2 is killed.
Note, posted interrupts require virtual interrupt deliveriy, and virtual
interrupt delivery requires exit-on-interrupt, ergo interrupts will be
unconditionally unmasked on VM-Enter if posted interrupts are enabled.

All the checks in lapic_timer_int_injected(), __kvm_wait_lapic_expire(), and
these function calls waste cpu cycles when the timer mode is not tscdeadline.
We can observe ~1.3% world switch time overhead by kvm-unit-tests/vmexit.flat
vmcall testing on AMD server. This patch reduces the world switch latency
caused by timer_advance_ns feature when the timer mode is not tscdeadline by
simpling move the check against apic->lapic_timer.expired_tscdeadline much
earlier.

The kick after setting KVM_REQ_PENDING_TIMER is used to handle the timer
fires on a different pCPU which vCPU is running on. This kick costs about
1000 clock cycles and we don't need this when injecting already-expired
timer or when using the VMX preemption timer because
kvm_lapic_expired_hv_timer() is called from the target vCPU.

Check apic_lvtt_tscdeadline() mode directly instead of apic_lvtt_oneshot()

There is missing apic map recalculation after updating DFR, if it is
INIT RESET, in x2apic mode, local apic is software enabled before.
This patch fix it by introducing the function kvm_apic_set_dfr() to
be called in INIT RESET handling path.

It is a little different between Intel and AMD, Intel's bit 2
is 0 and AMD is reserved. On bare-metal, Intel will refuse to set
APIC_TDCR once bits except 0, 1, 3 are setting, however, AMD will
accept bits 0, 1, 3 and ignore other bits setting as patch does.
Before the patch, we can get back anything what we set to the
APIC_TDCR, this patch improves it.

Beside called in kvm_vcpu_ioctl_set_cpuid*(), kvm_update_cpuid() is also
called 5 places else in x86.c and 1 place else in lapic.c. All those 6
places only need the part of updating guest CPUIDs (OSXSAVE, OSPKE, APIC,
KVM_FEATURE_PV_UNHALT, ...) based on the runtime vcpu state, so extract
them as a separate kvm_update_cpuid_runtime().

Guest fails to online hotplugged CPU with error
smpboot: do_boot_cpu failed(-1) to wakeup CPU#4
It's caused by the fact that kvm_apic_set_state(), which used to call
recalculate_apic_map() unconditionally and pulled hotplugged CPU into
apic map, is updating map conditionally on state changes. In this case
the APIC map is not considered dirty and the is not updated.
Fix the issue by forcing unconditional update from kvm_apic_set_state(),
like it used to be.

This patch implements a fast path for emulation of writes to the TSCDEADLINE
MSR. Besides shortcutting various housekeeping tasks in the vCPU loop,
the fast path can also deliver the timer interrupt directly without going
through KVM_REQ_PENDING_TIMER because it runs in vCPU context.

Replace the ad hoc test in vmx_set_hv_timer with a test in the caller,
start_hv_timer. This test is not Intel-specific and would be duplicated
when introducing the fast path for the TSC deadline MSR.

The use of any sort of waitqueue (simple or regular) for
wait/waking vcpus has always been an overkill and semantically
wrong. Because this is per-vcpu (which is blocked) there is
only ever a single waiting vcpu, thus no need for any sort of
queue.
As such, make use of the rcuwait primitive, with the following
considerations:
- rcuwait already provides the proper barriers that serialize
concurrent waiter and waker.
- Task wakeup is done in rcu read critical region, with a
stable task pointer.
- Because there is no concurrency among waiters, we need
not worry about rcuwait_wait_event() calls corrupting
the wait->task. As a consequence, this saves the locking
done in swait when modifying the queue. This also applies
to per-vcore wait for powerpc kvm-hv.
The x86 tscdeadline_latency test mentioned in 8577370fb0cb
("KVM: Use simple waitqueue for vcpu->wq") shows that, on avg,
latency is reduced by around 15-20% with this change.

kvm_vcpu_ioctl_get_lapic (implements KVM_GET_LAPIC ioctl) does a bulk copy
of the LAPIC registers but must take into account that the one-shot and
periodic timer current count register is computed upon reads and is not
present in register state. When restoring LAPIC state (e.g. after
migration), restart timers from their their current count values at time of
save.
Note: When a one-shot timer expires, the code in arch/x86/kvm/lapic.c does
not zero the value of the LAPIC initial count register (emulating HW
behavior). If no other timer is run and pending prior to a subsequent
KVM_GET_LAPIC call, the returned register set will include the expired
one-shot initial count. On a subsequent KVM_SET_LAPIC call the code will
see a non-zero initial count and start a new one-shot timer using the
expired timer's count. This is a prior existing bug and will be addressed

Replace the kvm_x86_ops pointer in common x86 with an instance of the
struct to save one pointer dereference when invoking functions. Copy the
struct by value to set the ops during kvm_init().
Arbitrarily use kvm_x86_ops.hardware_enable to track whether or not the
ops have been initialized, i.e. a vendor KVM module has been loaded.

This patch optimizes the virtual IPI fastpath emulation sequence:
write ICR2 send virtual IPI
read ICR2 write ICR2
send virtual IPI ==> write ICR
write ICR
We can observe ~0.67% performance improvement for IPI microbenchmark
(https://lore.kernel.org/kvm/[email protected]/)
on Skylake server.

The timer is disarmed when switching between TSC deadline and other modes,
we should set everything to disarmed state, however, LAPIC timer can be
emulated by preemption timer, it still works if vmx->hv_deadline_timer is
not -1. This patch also cancels preemption timer when disarm LAPIC timer.

In the vCPU reset and set APIC_BASE MSR path, the apic map will be recalculated
several times, each time it will consume 10+ us observed by ftrace in my
non-overcommit environment since the expensive memory allocate/mutex/rcu etc
operations. This patch optimizes it by recaluating apic map in batch, I hope
this can benefit the serverless scenario which can frequently create/destroy
VMs.
Before patch:
kvm_lapic_reset ~27us
After patch:
kvm_lapic_reset ~14us
Observed by ftrace, improve ~48%.

When pv_eoi_get_user() fails, 'val' may remain uninitialized and the return
value of pv_eoi_get_pending() becomes random. Fix the issue by initializing
the variable.

Even when APICv is disabled for L1 it can (and, actually, is) still
available for L2, this means we need to always call
vmx_deliver_nested_posted_interrupt() when attempting an interrupt
delivery.

Callers of kvm_apic_match_dest() should always pass in APIC_DEST_*
macros for either dest_mode and short_hand parameters. Fix up all the
callers of kvm_apic_match_dest() that are not following the rule.
Since at it, rename the parameter from short_hand to shorthand in
kvm_apic_match_dest(), as suggested by Vitaly.

We have both APIC_SHORT_MASK and KVM_APIC_SHORT_MASK defined for the
shorthand mask. Similarly, we have both APIC_DEST_MASK and
KVM_APIC_DEST_MASK defined for the destination mode mask.
Drop the KVM_APIC_* macros and replace the only user of them to use
the APIC_DEST_* macros instead. At the meantime, move APIC_SHORT_MASK
and APIC_DEST_MASK from lapic.c to lapic.h.

In IOAPIC fixed delivery mode instead of flushing the scan
requests to all vCPUs, we should only send the requests to
vCPUs specified within the destination field.
This patch introduces kvm_get_dest_vcpus_mask() API which
retrieves an array of target vCPUs by using
kvm_apic_map_get_dest_lapic() and then based on the
vcpus_idx, it sets the bit in a bitmap. However, if the above
fails kvm_get_dest_vcpus_mask() finds the target vCPUs by
traversing all available vCPUs. Followed by setting the
bits in the bitmap.
If we had different vCPUs in the previous request for the
same redirection table entry then bits corresponding to
these vCPUs are also set. This to done to keep
ioapic_handled_vectors synchronized.
This bitmap is then eventually passed on to
kvm_make_vcpus_request_mask() to generate a masked request
only for the target vCPUs.
This would enable us to reduce the latency overhead on isolated
vCPUs caused by the IPI to process due to KVM_REQ_IOAPIC_SCAN.

bitmap. Add helper func to remove it, and eliminate odd out label,
get rid of unnecessary kvm_lapic_irq field init and so on.

Commit 4b9852f4f389 ("KVM: x86: Fix INIT signal handling in various CPU states")
fixed KVM to also latch pending LAPIC INIT event when vCPU is in VMX
operation.
However, current API of KVM_SET_MP_STATE allows userspace to put vCPU
into KVM_MP_STATE_SIPI_RECEIVED or KVM_MP_STATE_INIT_RECEIVED even when
vCPU is in VMX operation.
Fix this by introducing a util method to check if vCPU state latch INIT
signals and use it in KVM_SET_MP_STATE handler.

Guest physical APIC ID may not equal to vcpu->vcpu_id in some case.
We may set the wrong physical id in avic_handle_ldr_update as we
always use vcpu->vcpu_id. Get physical APIC ID from vAPIC page
instead.
Export and use kvm_xapic_id here and in avic_handle_apic_id_update
as suggested by Vitaly.

5000 guest cycles delta is easy to encounter on desktop, per-vCPU
lapic_timer_advance_ns always keeps at 1000ns initial value, let's
loosen the filter a bit to let adaptive tuning make progress.
