Fix a double locking bug caused when debug.kprobe-optimization=0.
While the proc_kprobes_optimization_handler locks kprobe_mutex,
wait_for_kprobe_optimizer locks it again and that causes a double lock.
To fix the bug, this introduces different mutex for protecting
sysctl parameter and locks it in proc_kprobes_optimization_handler.
Of course, since we need to lock kprobe_mutex when touching kprobes
resources, that is done in *optimize_all_kprobes().
This bug was introduced by commit ad72b3bea744 ("kprobes: fix
wait_for_kprobe_optimizer()")

The BUG_ON() directive is triggered probably due to a latency
modification following inclusion of commit c10d73671ad3 ("softirq:
reduce latencies"). This condition has not been met before 3.9-rc1 and
doesn't trigger without this patch.
We now make sure that DMA channel is idle before calling
atc_complete_all() which makes the BUG_ON() "protection" useless.

The main additional change here is Lars-Peter's DMA work plus the
platform conversions which have been tested - getting this in mainline
will make life easier for development after the merge window. These

Use the generic dmaengine PCM driver instead of a custom implemention. There is
a minor functional change, the ux500 PCM driver did not preallocate the audio
buffer, while the generic dmaengine PCM driver will do this.

Unfortunately, none of the UAC standards provides a way to identify DSD
(Direct Stream Digital) formats. Hence, this patch adds a quirks
handler to identify USB interfaces that are capable of handling DSD.
That quirks handler can augment the already parsed formats bit-field,
by any of the new SNDRV_PCM_FMTBIT_DSD_{U8_U16} and setting the dsd_dop
flag in the audio format, if the driver should take care for the DOP
byte stuffing.
The only devices that are known to work with this are the ones with
a 'Playback Designs' vendor id.

There is quite some confusion around the bit-ordering in DSD samples,
and no general agreement that defines whether hardware is supposed to
expect the oldest sample in the MSB or the LSB of a byte.
ALSA will hence set the rule that on the software API layer, bytes
always carry the oldest bit in the most significant bit of a byte, and
the driver has to translate that at runtime in order to match the
hardware layout.
This patch adds support for this by adding a boolean flag to the
audio format struct.

In order to provide a compatibility way for pushing DSD
samples through ordinary PCM channels, the "DoP open Standard" was
invented. See http://www.dsd-guide.com for the official document.
The host is required to stuff DSD marker bytes (0x05, 0xfa,
alternating) in the MSB of 24 bit wide samples on the bus, in addition
to the 16 bits of actual DSD sample payload.
To support this, the hardware and software stride logic in the driver
has to be tweaked a bit, as we make the userspace believe we're
operating on 16 bit samples, while we in fact push one more byte per
channel down to the hardware.
The DOP runtime information is stored in struct snd_usb_substream, so
we can keep track of our state across multiple calls to
prepare_playback_urb_dsd_dop().

For normal PCM transfer, this change has no effect, as the endpoint's
stride is always frame_bits/8. For DSD DOP streams, however, which is
added later, the hardware stride differs from the software stride, and
the endpoint has the correct information in these cases.

This patch adds two formats for Direct Stream Digital (DSD), a
pulse-density encoding format which is described here:
https://en.wikipedia.org/wiki/Direct_Stream_Digital
DSD operates on 2.8, 5.6 or 11.2MHz sample rates and as a 1-bit
stream.
The two new types added by this patch describe streams that are capable
of handling DSD samples in DOP format as 8-bit or in 16-bit (or at a x8
or x16 data rate, respectively).
DSD itself specifies samples in *bit*, while DOP and ALSA handle them
as *bytes*. Hence, a factor of 8 or 16 has to be applied for the sample
rare configuration, according to the following table:

When pin default configs are overridden via patch option, these are
evaluated before fixups are applied. Since some fixups change the
whole codec trees and/or add pins dynamically, this sanity check might
not pass when pins aren't present at the time the function is called.
We may reorder the execution, but an easier fix is simply to disable
this sanity check.

This fixes a kernel memory contents leak via the tkill and tgkill syscalls
for compat processes.
This is visible in the siginfo_t->_sifields._rt.si_sigval.sival_ptr field
when handling signals delivered from tkill.

With applying the previous patch "hugetlbfs: stop setting VM_DONTDUMP in
initializing vma(VM_HUGETLB)" to reenable hugepage coredump, if a memory
error happens on a hugepage and the affected processes try to access the
error hugepage, we hit VM_BUG_ON(atomic_read(&page->_count) <= 0) in
get_page().
The reason for this bug is that coredump-related code doesn't recognise
"hugepage hwpoison entry" with which a pmd entry is replaced when a memory
error occurs on a hugepage.
In other words, physical address information is stored in different bit
layout between hugepage hwpoison entry and pmd entry, so
follow_hugetlb_page() which is called in get_dump_page() returns a wrong
page from a given address.
The expected behavior is like this:

With this patch, we can call hugetlb_fault() and take proper actions (we
wait for migration entries, fail with VM_FAULT_HWPOISON_LARGE for
hwpoisoned entries,) and as the result we can dump all hugepages except
for hwpoisoned ones.

Documentation/filesystems/proc.txt says about coredump_filter bitmask,
Note bit 0-4 doesn't effect any hugetlb memory. hugetlb memory are only
effected by bit 5-6.
However current code can go into the subsequent flag checks of bit 0-4
for vma(VM_HUGETLB). So this patch inserts 'return' and makes it work
as written in the document.

Currently we fail to include any data on hugepages into coredump,
because VM_DONTDUMP is set on hugetlbfs's vma. This behavior was
recently introduced by commit 314e51b9851b ("mm: kill vma flag
VM_RESERVED and mm->reserved_vm counter").
This looks to me a serious regression, so let's fix it.

hotkey_kthread() does try_to_freeze() under hotkey_thread_mutex.
We can simply kill this mutex, hotkey_poll_stop_sync() does not need to
serialize with hotkey_kthread(). When kthread_stop() returns the thread
is already dead, it called do_exit()->complete_vfork_done().

In the very unlikely event where a guest would be foolish enough to
*read* from a write-only cache maintainance register, we end up
with preemption disabled, due to a misplaced get_cpu().
Just move the "is_write" test outside of the critical section.

Vivek found old kexec-tools does not work new kernel anymore.
So change back crashkernel= back to old behavoir, and add crashkernel_high=
to let user decide if buffer could be above 4G, and also new kexec-tools will
be needed.

Chao said that kdump does does work well on his system on 3.8
without extra parameter, even iommu does not work with kdump.
And now have to append crashkernel_low=Y in first kernel to make
kdump work.
We have now modified crashkernel=X to allocate memory beyong 4G (if
available) and do not allocate low range for crashkernel if the user
does not specify that with crashkernel_low=Y. This causes regression
if iommu is not enabled. Without iommu, swiotlb needs to be setup in
first 4G and there is no low memory available to second kernel.
Set crashkernel_low automatically if the user does not specify that.
For system that does support IOMMU with kdump properly, user could
specify crashkernel_low=0 to save that 72M low ram.

It's possible that the reference to the object request dropped
inside the loop in rbd_img_request_submit() will be the last
one, in which case the content of the object pointer can't be
trusted.
Use a safe form of the object request list traversal to avoid
problems.

The flag bus->shutdown implies that the control elements might have
been already destroyed. When a codec is resumed at this state and
tries to call vmaster hook (e.g. in snd_hda_gen_init()), it would
refer to a non-existing object, resulting in Oops in the end.
This patch just adds a check of the flag in the caller side for
avoiding such a crash.
Though, the best would be to clear hook->sw_kctl by the destructor of
the corresponding ctl element, but vmaster uses its own private_free,
it can't be done easily. So let it be for a while.

Events may be created with attr->disabled == 1 and attr->enable_on_exec
== 1, which confuses the group validation code because events with the
PERF_EVENT_STATE_OFF are not considered candidates for scheduling, which
may lead to failure at group scheduling time.
This patch fixes the validation check for ARM, so that events in the
OFF state are still considered when enable_on_exec is true.

We must not declare dbg_cpu_pm_nb as __cpuinitdata as we need it after
system initialization for Suspend and CPUIdle.

On Feroceon the L2 cache becomes non-coherent with the CPU
when the L1 caches are disabled. Thus the L2 needs to be invalidated
after both L1 caches are disabled.
On kexec before the starting the code for relocation the kernel,
the L1 caches are disabled in cpu_froc_fin (cpu_v7_proc_fin for Feroceon),
but after L2 cache is never invalidated, because inv_all is not set
in cache-feroceon-l2.c.
So kernel relocation and decompression may has (and usually has) errors.
Setting the function enables L2 invalidation and fixes the issue.

tcm_init() call iotable_init() and it use early_alloc variants which
do memblock allocation. Directly using memblock allocation after
initializing bootmem should not permitted, because bootmem can't know
where are additinally reserved.
So move tcm_init() to a safe place before initalizing bootmem.

Pull networking fixes from David Miller:
1) Fix erroneous netfilter drop of SIP packets generated by some Cisco
phones, from Patrick McHardy.
2) Fix netfilter IPSET refcounting in list_set_add(), from Jozsef
Kadlecsik.
3) Fix TCP syncookies route lookup key, we don't use the same values we
would use for the usual SYN receive processing, from Dmitry Popov.
4) Fix NULL deref in bond_slave_netdev_event(), from Nikolay
Aleksandrov.
5) When bonding enslave fails, we can forget to clear the IFF_BONDING
bit, fix also from Nikolay Aleksandrov.
6) skb->csum_start is 16-bits, which is almost always just fine. But
if we reallocate the headroom of an SKB this can push the
skb->csum_start value outside of it's valid range. This can easily
happen when collapsing multiple SKBs from the retransmit queue
together.

7) Fix NULL deref in be2net driver due to missing check of
__vlan_put_tag() return value, from Ivan Vecera.
8) tun_set_iff() returns zero instead of error code on failure, fix

12) If userland specifies a path cost explicitly, do not override it

13) mvnets calculates the TX queue to use incorrectly resulting in
garbage pointer derefs and crashes, fix from Willy Tarreau.
14) cdc_mbim does erroneous sizeof(ETH_HLEN). Fix from Bjorn Mork.
15) IP fragmentation can leak a refcount-less route out from an RCU
protected section. This results in crashes and all sorts of hard to
diagnose behavior. Fix from Eric Dumazet.

Commit b4cbb197c7e7 ("vm: add vm_iomap_memory() helper function") added
a helper function wrapper around io_remap_pfn_range(), and every other
architecture defined it in <asm/pgtable.h>.
The s390 choice of <asm/io.h> may make sense, but is not very convenient
for this case, and gratuitous differences like that cause unexpected errors like this:

Using this parameter one can disable the storage_size/2 check if
he is really sure that the UEFI does sane gc and fulfills the spec.
This parameter is useful if a devices uses more than 50% of the
storage by default.
The Intel DQSW67 desktop board is such a sucker for exmaple.

The hardware revision of the codec is based at 0x40. Subtract that
before convering to ASCII. The same as it is done for 98095.

This patch adds a playback and capture streams to the dummy codec DAI
configuration. Most permissive set of sampling rates and formats is used.
This patch is needed for playback and capturing on a codec-less systems,
as otherwise the PCM device nodes are not even created.

This allows us to access the DAI DMA data when we create the PCM. We'll use
this when converting imx to generic DMA engine PCM driver.
