USB and PHY fixes for 4.5-rc4

Here are a number of USB and PHY driver fixes for 4.5-rc4.
 
 They are the usual gadget and xhci drivers that had reported problems,
 as well as a few small phy issues as well.  All have been in linux-next
 with no reported issues.
